Job Description:
Part time, Full time or Per Visit Associates degree required CPR Required Driver's License and Auto Insurance required Licensure Required two or more relevant experience Provides, coordinates, teaches, or supervises nursing care for a group of patients usually in the setting of the patient's home. Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services. In addition to specific and skilled treatment and procedures, the nurse provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Case Manager ensures adequate service provision, including scheduling of visits, based on patients' identified needs and priories for care. Complies with all Home Care policies and procedures.
Skills: * Ability to exercise independent judgement * Organized in approach to work accomplishment and meeting time requirements * Ability to communicate clearly and effectively with varied persons including patients, families, home care staff, physicians, and administrators * Ability to understand and comply with Agency and regulatory standards of care * Physical assessment skills * Phlebotomy * Understanding and ability to apply principles of teaching and learning to patients with varied abilities and needs * Ability to accept and work with individuals and families with varied social and cultural backgrounds without being judgmental * Safe, competent, and therapeutic hands-on nursing skills required * A high degree of flexibility and the ability to establish priorities * Use of sound judgment
EXPERIENCE: 2-3 years Med-Surg.; 1 year home care experience preferred.
EDUCATION: Accredited Nursing/BSN preferred.
